“Brad Morrison, Laura Rock, and I presented a workshop on Providing Effective Feedback at the American College of Rheumatology annual meeting in Washington, DC on November 21, 2016. Although the session was scheduled for an early hour, it was very well attended and the participants responded enthusiastically to the notions of Feedback with Good Judgment. We were able to provide some motivation, content, and practice for the participants improving their feedback to students, coworkers, and peers. Using some intriguing videos of good and bad feedback enabled us to make the session relevant to a medical specialty we have not traditionally served.”

CMS Hosts Alaskan Healthcare Providers

Last week, the Center for Medical Simulation hosted two Alaskan healthcare providers, Susan Davis, EMT, CHP-c, and Danita Koehler, MD. Susan and Danita came to CMS representing Eastern Aleutian Tribes, a healthcare corporation working alongside the Alaska Native Tribal Health Consortium to provide health services to communities in the Aleutian Islands and on the Alaska and Kenai peninsulas. These programs are part of the Alaska Community Health Aide Program, which provides resources to a network of more than 550 healthcare providers serving more than 170 rural villages throughout Alaska. ANTHC is Alaska’s second largest healthcare employer, with over 2,500 employees.

Susan and Danita attended the CMS Comprehensive Instructor Workshop in order to further their goal of bringing simulation training to these healthcare programs. The Community Health Aides / Practitioners who work in the CHAP program are the primary providers in their communities, working with limited resources to provide health education, prevent disease and injury, and ensure safe water and sewer services. These community providers work in conjunction with remote providers at hospitals, using teleconferencing to examine patients, discuss treatment options, and determine whether medevac resources are required in critical situations.

Simulation training will allow these frontline providers to safely train on and improve response times to many potential clinical situations, including early recognition of crucial signs and symptoms for issues like shock, sepsis, and psychiatric distress. Having spent a week at CMS training with an international and interprofessional team of healthcare providers, Susan and Danita are now better equipped to create, operate, and evangelize simulation programs for their healthcare organizations and their communities.

The Center for Medical Simulation is pleased to announce that both Jenny Rudolph, Executive Director of CMS, and Mary Fey, Associate Director of the Institute for Medical Simulation at CMS, will speak at the California Simulation Alliance’s second annual conference on October 15th, 2016, at the University of San Francisco.

Jenny and Mary will facilitate a special pre-conference debriefing workshop on the topic of Debriefing with Good Judgement©, on October 14th, 2016, with an introductory online webinar which took place on October 4th, 2016.

The theme of this year’s annual conference is “Making an Impact on Healthcare Education: Bridging the Gap Using Simulation.” Jenny Rudolph will deliver the keynote address at the conference, on the topic, “Creating a Transformational Learning Environment: High Standards and High Regard in Healthcare Simulation.” This interactive keynote speech will explore how to link holding learners to high standards while also holding them in high regard, as the basis for learning in simulation. Some debriefers are “nice” and withhold judgement to protect the learner, while others “tell it like it is” and demoralize the learner. Jenny will discuss how debriefers can do both these things simultaneously in a way that feels fair and fun.

 Mary Fey will also deliver a talk discussing the effects of the recent National Council of State Boards of Nursing (NCSBN) study on the use of simulation in nursing curricula. She will introduce the findings of the study, which showed that a certain percentage of contact hours for nursing students could be successfully and effectively replaced with simulation. She will also show nursing educators how to integrate simulation into their curricula, given the new NCSBN guidelines.

CMS Announces An Introductory Debriefing Workshop

The Gateway Debriefing Skills Workshop

The Center for Medical Simulation has developed an introductory debriefing workshop for healthcare educators titled “The Gateway Debriefing Workshop – An Introduction to Debriefing with Good Judgement.”  This introductory workshop introduces healthcare education faculty to the foundational practices of the “Debriefing with Good Judgment© method of debriefing. It combines a one day immersive in-person learning experience with online learning that includes customized peer to peer and mentor feedback.

Designed For Busy Healthcare Educators

Designed for the busy healthcare educator with limited time, the Gateway Debriefing Workshop immerses participants in the deliberate practice of debriefing. Participants will have the opportunity to:

  • Learn how to efficiently improve their debriefing performance
  • Learn how to set high standards while holding high regard for the learner
  • Treat learners as thinkers and not just doers
  • Practice various debriefing phases
  • Connect  to a community of colleagues interested in developing and improving their debriefing and feedback skills

The Workshop is intended to be an introduction to debriefing and can serve as a gateway to CMS’ multi-day Simulation Instructor Programs held at CMS in Boston and around the world. The Gateway Workshop will be facilitated by a lead CMS faculty member and a team of talented co-facilitators who have been trained by CMS.

Additional Details for Nursing Programs

Nursing programs whose faculty attend the Gateway Workshop will be able to provide evidence of meeting the National Council of State Boards of Nursing (NCSBN) guidelines.

Gateway Debriefing Workshop Details

Each Gateway Debriefing Workshop is able to enroll up to 50 participants and has two required components:

  • 2 hour online session held approximately 2 weeks before the workshop.
  • Day long In-person workshop held at the Center for Medical Simulation in Boston and at host sites throughout the United States.

Participants will also be provided with the opportunity to connect with a growing community of simulation colleagues where they can continue to develop and improve their debriefing and feedback skills.

Healthcare providers will receive a “Certificate of Attendance” with credit hours that can be used to apply for continuing education credits.

With an emphasis on building humane, learning-oriented cultures in healthcare, Dr. Rudolph creates, studies and writes about feedback and debriefing conversations to help people analyze and improve their skills as clinicians and educators.

Dr. Rudolph is an assistant professor of anaesthesia at Harvard Medical School and the Department of Anesthesia Critical Care and Pain Medicine at Massachusetts General Hospital.  

Dr. Rudolph received her doctorate in organizational behavior from Boston College, Carroll School of Management, and her B.A. in sociology from Harvard College.

Debriefing clinical simulation experiences is increasingly understood as a crucial step in clarifying and consolidating insights and lessons from simulations. CMSDASH is designed to assist in evaluating and developing debriefing skills. It evaluates the strategies and techniques used to conduct debriefings by examining concrete behaviors. The DASH is based on evidence and theory about how people learn and change in experiential contexts, and is designed to allow assessment of debriefings from a variety of disciplines and courses, varying numbers of participants, a wide range of educational objectives, and various physical and time constraints. Previous participants in the rater training have said they find it very useful in providing feedback to debriefers or formally assessing their debriefing skills.

Jenny Rudolph talks about how excellent debriefing or interviewing is sometimes invisible

jenny-rudolphWhen I watch or listen to interviews I’ve noticed something.  In excellent interviews like in excellent debriefings, the questions and questioner seem to fade into the background and the story and ideas of the learner or interviewee stand out.  Like a well-designed setting for a precious gem, or good lighting in the theater, good debriefing and interviewing is not that noticeable.  This is ironic because it takes some effort and practice to master the interview and debriefing techniques to be invisible. 

A lovely 2.5 minute example of this is Joshua Gubitz’s interview of his 85 year-old friend Len Berk.  When Berk retired as a CPA he got himself hired as a salmon slicer at a gourmet deli in NYC.  Although this clip no doubt benefitted from skillful audio editing, you will notice that Josh Gubitz just taps the topic along, with little follow-up questions, or a chuckle of encouragement.  He doesn’t get in the way of Berk telling his story.  Gubitz’s underlying feeling of interest and positive regard for Berk and Berk’s inherent love of salmon slicing do the rest.
