CMS Marketing Release

I, the undersigned participant, hereby grant permission to the Center for Medical Simulation (“CMS”) and its representatives, employees, agents, and assigns to record, photograph, film, and otherwise capture my name, likeness, image, voice, statements, presentation materials, and participation in connection with educational, promotional, and marketing activities, including but not limited to podcasts, interviews, webinars, livestreams, social media content, websites, digital publications, and other audio-video productions (collectively, the “Recordings”).

I understand and agree that CMS may edit, reproduce, publish, distribute, display, broadcast, stream, transmit, archive, and otherwise use the Recordings, in whole or in part, in any format or medium now known or later developed, worldwide and in perpetuity, without further notice, review, approval, or compensation to me.

I acknowledge that the Recordings may be publicly accessible and may be shared, reposted, excerpted, commented upon, or redistributed by third parties outside the control of CMS. I understand that CMS cannot guarantee how external parties may use or interpret the content once publicly released.

I represent that my participation and any materials or statements I provide do not knowingly infringe upon the rights of any third party, including copyright, confidentiality, privacy, or trademark rights.

Release and Indemnification

To the fullest extent permitted by law, I release and hold harmless CMS, its officers, directors, employees, faculty, contractors, affiliates, successors, and assigns from any and all claims, demands, liabilities, damages, losses, costs, or expenses arising out of or related to the creation, publication, distribution, transmission, or use of the Recordings, including but not limited to claims relating to defamation, privacy, publicity rights, emotional distress, reputational harm, or unauthorized third-party use or redistribution of the content. I further agree to indemnify and defend CMS against any third-party claims arising from materials, statements, or content that I knowingly provide in connection with the Recordings.

I understand that participation is voluntary and that I may discontinue participation at any time prior to publication or release of the final edited content.

By signing below, I acknowledge that I have read and understood this release and voluntarily agree to its terms.
